Medication: Teachers cannot legally diagnose, prescribe or treat. Teachers may administer medication to students ONLY when the procedure below is carefully followed.

  1. A written request from the parents indicating that medication be given to their child.
  2. A written order from the attending physician stating the name of the medication, dosage, time to be given, and medication effect.
  3. Medication be given directly to the homeroom teacher or school secretary upon arrival.
Sick Children: When a child has a high temperature, diarrhea, throws up or is generally feeling very unwell, the parents will be contacted to pick up their child from school.

Lice Policy: When a student is found to have lice, the parents must contact the school immediately. A head check on students and staff may be done at that time. A student with lice may return to school only when they have been fully treated. Written permission may be required from their doctor.

Physicals: Physicals are required upon entrance of 1st, 5th and 9th grade, as well as any student coming in from outside of Wisconsin.

Immunizations: The current Wisconsin State Public Health Law requires that all school children be immunized as follows for the 2006-2007 school year:

Grades K - 12
4 DTP/DTaP/DT 3
4 Polio 4
2 MMR 1
3 Hep B
1 Var 5,6
We have an immunization form you can fill out online, then print it, have it signed by a doctor and a parent/legal custodian/guardian/Adult student. Click here (this is an Adobe Acrobat form, if you have trouble get the latest version here)
A physician's certificate or previous school record must be presented proof of immunization.
